Insecticide/Fungicide concoction help.

A few years ago, there was a great discussion about an insecticide/fungicide concoction for tomatoes and other veg plants.  It consisted of Daconil; Ferti-lome borer, bagworm, caterpillar spray; Vegetable plus, and spreader sticker.

Does anyone happen to have the ratio for each in the solution.  I need to refill my pump sprayer and I've lost my notes somewhere.  Any help is appreciated.

Per gardendoc
Per gallon:
1 tbs permetheran 10%
1/4 cup (4 tbs) spinosad
3 tbs daconil
2 tbs (1 oz) spreader sticker

I made this into a document, framed it and hung it where we keep the sprayer and the chemicals.
